Fixed logic error in opportunity controller saving / removing an
account. Now, to remove an account, the param must be :account
=> {}. If no account param is given, don't touch the saved
account. Added spec for this case. Also fixed a typo, and changed
tests to use 'Setting.default_access' instead of assuming
'Private'
